Here are common questions from candidates about the background investigation stage of our hiring process.
The positions subject to a background include firefighter, paramedic, fire prevention roles, firefighter interns, and incident support volunteers.
Oregon law (OAR 259-009-0015) requires fire agencies to conduct a thorough background investigation on candidates being considered for fire service roles. Our process and the supporting materials we request are guided by this statute. We will provide additional, robust direction throughout the entire testing and selection process, including the background investigation.
| What is a Personal History Statement (PHS)? |
A Personal History Statement or PHS is a form that requires the compilation of biographical information and documents. It is the first step in the background investigation process. Please note: The PHS to complete is linked at the bottom of this page. It is a different form than the Personal History Questionnaire (PHQ) found on the National Testing Network (NTN) website. It is important to start on your PHS early. The PHS is a very thorough document, and it will take time to compile the information you need. You may need to request copies of some documents and have them mailed or e-mailed to you. |
| What documents and information will I need to gather and provide to my background investigator? |
This should not be considered an exhaustive list of the documents needed. Your background investigator may ask you to provide additional documents or information during the course of your investigation. Candidates invited to the panel interview must gather these items. You will submit them immediately upon notification of movement to the background investigation (if advancing). You will need to provide:

| When does the background investigation occur? |
The background investigation occurs after the panel (“Chief’s”) interview. Candidates on the eligible list who are invited to interview should immediately begin gathering their documents and completing their PHS form (see below for hyperlink to form). Those advancing forward after the panel interview will receive an invitation to the full background investigation. At that time, you will submit your PHS and supporting documentation to your assigned background investigator. You will also need to sign your Authorization to Release & Waiver of All Claims in front of a notary. |
| Where can I get my Authorization to Release & Waiver form notarized? |
The National Testing Network, which conducts the background investigation, offers virtual notary services for a fee. They will provide instructions and the fee amount to candidates who must submit this form. Notary services are sometimes available at banks, libraries, and some businesses. Mobile notaries will come to your home or workplace. A fee is often associated with these services. |
| When will I receive notification that my background investigation is complete? |
