It's important responders know about your conditions and medications.

Complete an Emergency Medical Card (PDF - English and Spanish) and post it on your refrigerator. 

This card provides responders with critical information such as current medications, any existing health conditions, physician’s name, and emergency contacts. You can also include the location of advanced directives including Do Not Resuscitate (DNR) orders and Portable (or Physician) Orders for Life-Sustaining Treatment (POLST) documents.
